1. How often do you contact your GP surgery?

2. How easy do you find it to get through to your GP surgery on the telephone?

3. How helpful do you find the staff at your GP surgery?

4. Are you happy for our trained staff to redirect you to a more appropriate service when possible, in your best interest? For example, the surgery pharmacist, physiotherapist, mental health nurse etc? 

5. Are you happy that when clinically safe your query can be dealt with during a telephone consultation with your GP rather than a face-to-face appointment?
